Elderly Disabled
Waiver Program
Also known as the Community Care Services Program (CCSP) is a case management program that serves the frail, elderly, and disabled who would benefit from nursing home level of care but wish to remain in their homes and/or communities. Through case management and service coordination, individuals are able to remain in the community and avoid being placed in a long-term care institution.

Worn in case of medical emergencies. This provides in-home electronic support system providing two- way communication between isolated persons and a medical control center which is available 24 hours a day, seven days a week.
This service provides in-home respite care provided by an aide. I also includes assistance with meal preparation, hygiene, nutrition, light housekeeping, necessary errands, and other support service...
