Waiver Program
Also known as the Community Care Services Program (CCSP) is a case management program that serves the frail, elderly, and disabled who would benefit from nursing home level of care but wish to remain in their homes and/or communities. Through case management and service coordination, individuals are able to remain in the community and avoid being placed in a long-term care institution. Those who are currently receiving hospice care can participate in the EDWP program with careful and deliberate collaboration between the two programs.

Day Health
Our program offers recreational and social activities, personal care and grooming assistance, meals and snacks, and exercise opportunities, all overseen by our nursing staff.
Living
For individuals who benefit from 24-hour care, we offer Alternative Living Services, such as personal care homes. These residences provide a safe and supportive environment.
Delivered Meals
A service designed to provide nutritious meals to individuals who may have difficulty preparing food for themselves due to age, disability, or illness.
Health Services
We Deliver nursing and/or therapy services right to your home. This includes personal care, assistance with meals, and other essential health services.
Response System
This provides in-home electronic support system providing two-way communication between isolated persons and a medical control center which is available 24/7.
Support Services
This service provides in-home respite care provided by an aide. I also includes assistance with meal preparation, hygiene, nutrition, and other support services
Management
We will guide you through all assessments, care coordination, service monitoring, education and resource management.
Family Caregiving
Support, education and oversight of family caregivers
that live in the home.

Home Health Services
Our Home Health Services deliver nursing and/or therapy services right to your home. This includes personal care, assistance with meals, and other essential health services, ensuring you receive the care you need in the comfort of your own home.
